Clinical context
A 65-year-old smoker presents with chronic cough and copious purulent sputum production. Chest CT reveals cylindrical and varicose dilatation of bronchi in the lower lobes. The patient has a history of recurrent respiratory infections and was treated for tuberculosis 10 years ago. Physical examination shows clubbing of fingers and coarse crackles at the lung bases.
