Clinical context
A 58-year-old man with type 2 diabetes mellitus presents to the clinic with progressive numbness and burning pain in both feet that has worsened over the past 6 months. He describes the sensation as 'like wearing tight shoes' and reports difficulty walking due to loss of sensation. Physical examination reveals decreased light touch sensation and vibration sense in a stocking distribution bilaterally.
