hepatorenal syndrome
Definition
A condition characterized by the simultaneous dysfunction of both liver and kidney function, often seen in advanced liver disease
Clinical context
A 58-year-old man with cirrhosis presents to the emergency department with worsening confusion and decreased urine output. His serum creatinine has risen from 1.2 to 3.8 mg/dL over the past 3 days, and he has developed ascites that is refractory to diuretic therapy. His hepatorenal syndrome is likely precipitated by spontaneous bacterial peritonitis. He requires urgent evaluation for liver transplantation.
