Separation of the nail plate from the nail bed
Clinical context
A 28-year-old woman presents with progressive separation of her fingernails starting from the distal edge, with the nail plate appearing white and lifted from the nail bed. She reports no pain but notices the affected nails have become thickened and deformed over several months. On examination, the nail plate is easily separable from the nail bed with minimal pressure.
