Clinical context
A 65-year-old male presents to the emergency department with headache, dizziness, and visual disturbances. On examination, his blood pressure is elevated at 180/100 mmHg and he has a ruddy complexion with engorged neck veins. Laboratory studies reveal a hematocrit of 65% and elevated red blood cell count.
