Definition

A condition characterized by excessive water retention due to inappropriate secretion of antidiuretic hormone, leading to dilutional hyponatremia

Clinical context

A 65-year-old man presents to the emergency department with confusion and seizures. He has been taking lisinopril for hypertension and reports feeling nauseous and weak for the past few days. His serum sodium is 128 mEq/L, urine osmolality is 500 mOsm/kg, and serum osmolality is 250 mOsm/kg. He has no signs of heart failure or renal disease. The most likely diagnosis is SIADH.
