Clinical context
A 28-year-old woman presents to the emergency department with difficulty swallowing and shortness of breath that has worsened over the past week. On examination, there is a visible bulge in her neck and she appears anxious with palpitations. Her vital signs show tachycardia and elevated blood pressure. The physical exam reveals a goiter with palpable nodules. Laboratory studies show elevated TSH and free T4 levels. The patient's symptoms are consistent with thyromegaly due to hyperthyroidism.
